All new clients are required to complete a Good Faith Exam. Please schedule new patient visit and add each service you would like. Returning patients will only need to schedule for each service.
Weightloss patients will need to schedule Good Faith Exam initially, before starting 3rd month and before starting 5th month.
Cancellations: We understand life happens and my may need to cancel or reschedule your appointment - Please provide 24 hours' notice.
